Purchase and Renewal Conditions: 
Joining a Prodigy Health RX plan signifies your agreement to our terms. By subscribing, whether for yourself or a minor, you confirm being over 18 and authorize Prodigy Health RX to charge your payment method. Your plan renews automatically, and charges apply until you cancel in writing. Your agreement signifies understanding and acceptance of these conditions.

Termination Conditions: 
Prodigy Health RX reserves the right to terminate memberships for reasons such as non-payment. If termination occurs for reasons other than non-payment, you will receive a proportional refund of your membership fees.

Cancellation Conditions:
Cancel within the first 30 days after joining for a full refund, less any processing fees. To cancel and request a refund, send a written notice. We process cancellation requests promptly, ceasing fee collection within 30 days of notice. Continued access to the plan remains until the end of the paid period. Specific conditions apply for certain states regarding refunds.

Description of Services:
Refer to the enclosed materials for detailed descriptions of the services included in your plan. Limitations, Exclusions & Exceptions: Prodigy Health RX offers a discount membership program. We are not an insurer or health service provider. We cannot guarantee pharmacy continuity.
Complaint Procedure: Submit any complaints in writing to Prodigy Health RX. An appeal process is available for unresolved complaints. Contact details for your state insurance department will be provided for further dissatisfaction.

ONLINE APP Terms & Conditions:
This plan is NOT INSURANCE and is not a substitute for health insurance. It does not fulfill minimum creditable coverage requirements for health insurance under any law. It's not a Qualified Health Plan under the Affordable Care Act, nor a Medicare prescription drug plan. Discounts vary by provider and service. Members must pay providers but receive discounts from participating providers. The plan does not make direct payments. You can cancel within 30 days of joining for a full refund, minus a nominal processing fee.
